Packeteer Equipment Move Form

All Packeteer equipment location changes must be reported to Packeteer, within 10 business days, to ensure your service contract remains in effect. If you have any questions please contact Customer Support for assistance.

Please complete the following form for each unit that has changed locations. If more than one unit has moved and shares the same physical location, please be sure to enter each serial number. If multiple units have moved to multiple locations a separate form must be completed for each address.
